• LEGALIZATION REQUIREMENTS: The original document must have an original signature. The signature must be notarized by a commissioned U.S. Notary Public.
  

• PROPER NOTARIZATION REQUIRED:  The Notary Public should comply with all state notarization requirements. The minimum requirements are:
  

- The venue ("State of ....,  County of....") must be indicated.

- The date of the notarization must be included. 

- The Notary's signature must be original.
